What area do we cover?

We provide services for Hopton, Stanton and many surrounding villages both in Suffolk and Norfolk. The receptionist will tell you if your address is in our practice area. We are also sometimes able to accept patients residing in Rushford.

How do I register?

If you wish to register with the practice, you will need to complete a registration form and we ask that you complete a medical questionnaire.

Your NHS Number

If you already have an NHS Number, please include it on the form as this can reduce delays and helps ensure you existing medical records can be traced.

Ways to find your NHS Number:

  1. The Find my NHS Number service (You must be currently registered with a GP surgery and have your mobile number or email address recorded with them)
  2. Documents from a former GP surgery such as letters or prescriptions
  3. Contact your current GP surgery

Running out of medication?

If you have regular medication and are still registered with a GP surgery elsewhere, please contact your current surgery. You can ask them to send a final prescription electronically and they can either send this to a specific pharmacy of your choosing or you can ask them to leave it "on the NHS Spine". Leaving your prescription on the Spine means you can collect your medication from any pharmacy within England. Note that the pharmacy cannot prepare it before you contact them; if you know it, have your NHS number with you as this can help the pharmacy find your electronic prescription. Once you are registered with us, our dispensing team can help with your medication needs if you live more than 1.6km from a pharmacy.

Preferred Practitioner

All patients are now registered with the practice so you may be seen by any doctor. You have a right to express a preference to receive medical services from a particular member of the clinical team and your preference will be recorded in your medical notes. However, there may be reasonable grounds when it is not possible to see the clinician of your choice.

Pre-registration

If you wish to pre-register, click on the link below to open the form. When you have completed all of the details, click on the 'Send' button to mail your form to us. You still need to visit the surgery to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
